PA School Letters of Recommendation: What Actually Matters
Letters of recommendation are the part of the application that does not get talked about much because applicants do not write them, and the part that quietly sinks otherwise strong files at meaningful rates. Here is who to ask, when to ask, and how to make sure the letter that gets written is the strongest possible version of itself.
What CASPA requires
CASPA requires a minimum of two letters of recommendation. Most programs require three, and some programs strongly prefer specific letter types (one from a PA, one from a science professor, one from a clinical supervisor). The cleanest approach is to plan for three letters from the start.
Who to ask, in order of weight
- A practicing PA-C who has worked with you clinically. The strongest single letter you can have. The PA can speak to your clinical aptitude and to your understanding of the role specifically. Programs explicitly weight this.
- A clinical supervisor (MD/DO/RN/NP) who has supervised you in a PCE role. Speaks to clinical aptitude. Almost as strong as the PA letter for any program that requires a clinical supervisor.
- A science professor (preferably one who taught you in a small class). Speaks to academic capacity. Strong if the professor can speak to you specifically rather than as a face in a 200-person lecture.
- A non-science professor (only if needed for a third letter). Acceptable but weaker than science. Usually only used to fill a third slot.
- A non-clinical work supervisor. Acceptable as a third letter if the supervisor can speak to leadership, work ethic, or skills relevant to clinical work.
Avoid: family members, family friends, religious leaders unless they are also clinical supervisors, anyone who cannot speak to your work specifically.
When to ask
By April for a June 1 CASPA submission. The two-month buffer matters because letter writers go quiet, get busy, take vacation, lose track of the deadline. CASPA verification will not begin until your letters arrive, and a letter that arrives in late June can delay verification by a month at the worst possible point in the cycle.

What to give the writer
A complete LOR packet for each writer makes the letter dramatically easier and stronger. Send:

- The list of programs you are applying to.
- Specific examples of your work together (a project, a patient case, a course paper, etc.) that you would like the writer to draw on.
- The CASPA submission instructions and the deadline.
- A brief paragraph on what aspects of your application you would value the writer addressing.

The failure modes
Late letters
The most common cause of cycle damage from LORs. Verification does not start until two letters arrive. Late verification puts the file in the late review pile. A friendly check-in email at three weeks and again at five weeks usually keeps the writer on track.
Generic letters
A letter that says "X is a hard worker and would make a great PA" without specifics reads as boilerplate. Programs notice. The fix is the LOR packet - give the writer specific moments to draw on.
Letters from people who do not know you well
A 2-sentence letter from a famous physician is worse than a 2-page letter from a less famous PA who knows you. Title does not substitute for specificity. Pick people who can write about you specifically.
Lukewarm letters
A letter that praises faintly is sometimes worse than no letter at all. If you are unsure whether a writer will be enthusiastic, ask: "Are you in a position to write me a strong letter for PA school?" The phrasing gives the writer permission to decline. A "let me think about it" or "I am busy this month" is usually a soft no, and it is worth taking.
What I tell applicants who are stuck on writers
If you do not have a PA who knows you, find one. Cold-email a PA in your area, ask if you can shadow for ten hours, do the shadowing well, and at the end ask about a recommendation. The shadow exposure compounds with the PCE work that counts if you are still building hours. Most PAs say yes. The PA letter is the single strongest letter type in the file, and the cost of acquiring one is two weeks of email and a few shadowing shifts. That is one of the highest-ROI moves available before submission.
If your science professor letters are weak because you came up through a large state school where you did not have small classes, find a graduate-level science course at a community college or post-bacc program before applying. Take the course, do well, get to know the professor, and ask for a letter at the end. Same logic - the cost is one course; the benefit is a specific letter from someone who knows your academic work.
